- Find a DQ Location in your area
- Drive up and chow down on some ice cream
- Save the receipt as you will need the DQFanSurvey Code
- Visit www.dqfansurvey.com
- Enter the 19 digit survey code
- Provide feedback
- Collect the free Dilly Coupon
Also, note the DQFanSurvey Free Dilly Bar Survey is operated by SMG (aka Service Management Group).
Hence, all feedback provided by DQ customers will be sent to the general managers for review.
Unfortunately, the only way to receive the free Dilly Bar is by taking the survey.

Yes, the survey is available to Canadian residents as well. Just visit www.dqfanfeedback.com to take the survey and select the French language option (if necessary).
Customers who would like to check out the DQ Canada menu can visit www.dairyqueen.com/ca-en.
To be honest the DQ Canada menu is pretty much the same as the US.
