The Visionworks of America Eyewear survey sweepstakes is open to legal residents residing in the 48 contiguous United States and D.C. who are at least the age of majority in their state of residence. Employees of Visionworks of America and their immediate family members are NOT allowed to enter.

Those who made a purchase and would like to enter with receipt should visit www.eyewearsurvey.com and enter the store number and transaction number located on the Visionworks receipt (the customer will be asked to complete the eight-question survey before they can enter the sweepstakes).
