Gas Visit Survey Sweepstakes
Win a $25 Phillips 66, Conoco or 76 gas card by taking a short survey in regards to a gas purchase. No purchase is required to enter and employees of Phillips 66 Company (“Sponsor”), Market Force Information, Inc., and SCA Promotions, LLC are not allowed to enter. In order to take the survey online the customers will need the zip code from the Phillips 66, Conoco or 76 customer receipt in order to begin. After the customer has completed the survey they will have the chance to enter the sweepstakes (once the entry is received a confirmation that they have entered into the Sweepstakes will appear).
Customers who would like to enter without making a costly gasoline purchase can write to: Phillips 66 Customer Satisfaction Survey Entry, PO Box 10490 Rochester, NY 14610. Please include a 3 x 5 index card with the contestants name, mailing address (no PO Boxes and include city, state, zip), phone number, email address, and age.

- Potential survey sweepstakes winners will be selected by a random drawing on or before 5:00 p.m. EST on the Winner Selection Date described above from among all eligible entries received by Sponsor
- Prizes will be fulfilled by regular U.S. mail within fifteen (15) business days of winner verification
- The gift card drawings will be administered by SCA Promotions
