The Super1Foods Guest Feedback Survey is open to legal residents of Texas, Arkansas, or Louisiana who are 18 years of age or older. Employees of Brookshire Grocery Company are NOT allowed to enter.
In order to enter the promotion without making a purchase simply write to: Guest Survey Monthly sweepstakes, PO Box 1411, Tyler, TX 75710. Please include the contestants name, address, phone number, date of birth, and email address on a 3 x 5 index card with proper postage.
Super1Foods Guest Survey Monthly Sweepstakes Promotional Notes
- A must enter survey sweepstakes for those who shop at Super1Food, Brookshire Food Stores, FRESH, or Spring Market stores
- The $500 gift card cannot be traded in for cash
- A list of winners will be made available after each $500 gift card drawing
- There is not a limit to the amount of times a contestant can enter the Super1FoodFeedback survey sweepstakes by mail
- Drawings will normally take place on or around the 10th day of the month for all entries in the prior month
- Non winning Super1FoodsFeedback Survey Sweepstakes entries will not be carried over to the next month sweepstakes
- Total value of all gift cards to be given away will equal $5000
- Lucky $500 gift card winners will be notified by email and or phone
- All winners will receive a IRS form 1099 to report their winnings to the IRS come tax time!
