In order to start the survey simply visit www.mycfavisit.com and provide the serial number from the Chick-Fil-A customer receipt.
Those who do not have the survey serial number will not be able to take the MYCFAVisit Survey (and will not earn the free Chicken Sammy!). Assuming the survey is completed in full the customer will be emailed a code within the next 24 hours for a free Chicken Sandwich.
