The MyZaxbysVisit Survey Sweepstakes is open to legal residents of the U.S., who are 18 years of age or older.
How to enter?
With purchase: Make a purchase at Zaxby’s and then visit www.myzaxbysvisit.com. The customer will have to provide the time of visit, amount of money spent, and the survey entry code.

Without purchase: Please send the contestant’s first name, last name, home telephone number, full address (no PO Boxes accepted), email address, and the name of a participating client to: [Enter Month/Year]Mail-In Entry c/o Empathica Weekly Sweepstakes c/o [Enter Name of Participating Client], 511 Avenue of the Americas, #40, New York, NY 10011. The contestant’s information should be written on a 3 x 5 piece of paper.

- The Zaxby’s survey code can be found on the customer receipt and can only be obtained via purchase
- Please note the MyZaxbysVisit sweepstakes is sponsored by Empathica, Inc. DBA and InMoment across multiple international clients
- The prize will be sent to the winner within 60 days of the draw date
- A purchase is not required in order to enter, and a purchase will not increase the odds of winning the $1,000 cash prize
- Winners will be responsible for any taxes associated with the cash prize
