The Perkins Experience Survey is open to legal residents of the United States who have recently visited a Perkins Restaurant & Bakery. Customers can take the survey with the 15 digit survey code OR by providing the store number, check number, date and time of purchase.
In order to complete the survey the and earn the 10% off coupon the customer will have to answer a few short questions in regards to their Perkins Restaurant & Bakery visit. Most questions should center around the friendliness of the service and quality of the food.
In order to take the survey simply visit www.perkinsexperiencesurvey.com and provide the 15 digit code (Internet connection is required as the survey cannot be taken by phone or mail).

- The 10% off validation code can be used at any Perkins location
- Those who do not have the receipt will not be able to take the Perkins Experience survey

True Perkins Restaurant & Bakery FAN BOYS will want to sign up for the My Perkins Rewards Program which will provide the customer with a 20% off coupon upon initial sign up. After initial sign up members will receive an annual gift on their birthday and ongoing promotions by email. To sign up for the My Perkins Rewards Program the customer will have to provide their name, email address, address, and the location of the Perkins in which they visit most.
