My Nike Visit Canada Survey

The MyNikeVisit Canada Survey is open to consumers who have recently visited a Nike retail location and have made a purchase. Please do not toss the receipt in the trash as the survey entry code from the customer receipt will be needed (customers who accidentally tossed the receipt in the trash may have some dumpster diving to do in order to take the survey assuming it is not covered in last nights pasta sauce).
In order to take the survey please visit www.mynikevisit-can.com and provide the 15 or 20 digit survey entry code provided on the receipt.
Please note the MyNikeVisit Promotional Gift Code is redeemable for merchandise at any NIKETOWN, NikeStore, Nike Factory Store, or Nike Clearance Store. Contrary to popular belief the code is not redeemable on telephone orders or online orders at Nike.com.

- A must take survey coupon due to the valuable coupon
- Limit to 2 completed surveys per household in a 7 day period
- The code balance is redeemable for merchandise only, and will not be redeemed for cash (except in accordance with applicable law), toward previous purchases or with any other offer, including employee discount
- If the survey code is lost Nike will not issue a replacement
- Only one $5 MyNikeVisit Canada coupon can be used per visit and the coupon cannot be combined with any other promotions
- The survey is available in four languages
