Destination Maternity Feedback Prize Notes

1. A total of twelve (12) Grand Prizes will be awarded; one (1) prize per Monthly Entry Period’s random drawing
2.  Each Grand Prize winner (each referred to hereafter as a “winner”) will receive a $500 gift card
3. Maximum ARV of all prizes: $6,000.00
4. Each gift card awarded is subject to the terms and conditions governing it and is non-transferable and non-assignable except to a surviving spouse
5. The prize is not redeemable for cash
6. No substitution for any prize is permitted except if a prize is unavailable
